TeleMental Health Examination

The TeleMental Health Examination (TMHE) is administered as part of the certification process that independently verifies a candidate's knowledge of core telemental health best practices. It was created from the consensus of subject matter experts in telemental health services and normed on a population of professionals in the field. The examination covers the following competency areas:

  1. Orientation to TeleMental Health
  2. Practitioner Protocols and Informed Consent
  3. Implementation of Telemental Health
  4. Professional, Legal, and Compliance Standards
  5. Professional Ethics
  6. Crisis Management

The TMHE is composed of 12 case studies comprised of 10 multiple-choice questions each. The case studies present hypothetical clients who are receiving TMH services and other scenarios involving the implementation of TMH. Each case study assesses a candidate’s ability to provide care in a safe and effective way. Of the 120 examination items on the TMHE, 100 items are scored and 20 items are unscored for field testing. Both scored and unscored items are of the same structure, and they appear in a randomized order throughout the examination. Examinees will have 3 hours to complete the TMHE.

The cut score (minimum passing score) for the TMHE is calculated through standard setting, which requires TMHE subject matter experts to review and evaluate each question on the examination to determine the passing score that would be expected from a Minimally Qualified Candidate (MQC). A process of statistical equating is used to determine the cut score for each version of the examination. Statistical equating ensures fairness to all candidates by associating the cut score on a test form with the overall difficulty level of the items on it.
